Micro-TESE (Microsurgical Testicular Sperm Extraction)

Micro TESE surgery (azoospermia treatment) is an advanced surgical procedure applied to search for sperm from testicular tissue using a microscopic method in men who have no sperm in their semen (azoospermia).
In this procedure performed under general anesthesia in cases of non-obstructive azoospermia, the testicular tissue is examined in detail under a microscope and tissue samples are taken from regions where sperm production exists.
The obtained sperm can be frozen to be used in fertilization with the microinjection (ICSI) method.
FertiJin Women’s Health and IVF Center in Istanbul Şişli stands out with its high success rates and experienced team in micro TESE operations.

What is Micro TESE?

Micro TESE (Microscopic Testicular Sperm Extraction) is the procedure of directly obtaining sperm from testicular tissue in men who have no sperm in their semen.
In this method, the testis is examined by magnification under a microscope and tissue samples are taken from regions containing sperm cells.
If sperm is found in the first application, these sperm can be frozen and used in future processes.

Who is Micro TESE Applied To?

  • In men who have no sperm in semen (azoospermia),
  • In cases of non-obstructive (non-obstructive) azoospermia,
  • In men whose suitability is determined by genetic and hormonal tests,
  • In patients who could not obtain results with classical TESE before.

Note: In case of deficiency of gene regions on the Y chromosome, sperm production may not occur. Therefore, it is of great importance to perform genetic testing and hormone evaluation before surgery.

How is Micro TESE Surgery Performed?

The Micro TESE operation takes an average of 1–2 hours under general anesthesia.
The testicular capsule is opened and enlarged sperm ducts are examined one by one with the help of a microscope, and tissue is taken from regions containing sperm.
These samples are sent to the laboratory and mature sperm are searched by embryologists.
The found sperm are frozen to be used in IVF or ICSI procedures.

Success Rates

The sperm retrieval rate with Micro TESE varies between 40% – 60% depending on the patient’s condition.
In cases where sperm cannot be found in the first application, the sperm retrieval rate may increase by 10% – 30% in the second TESE surgery with medication support.

Things to Consider After Surgery

  • Ice application should be applied to the area during the first 24–48 hours after surgery.
  • Antibiotic and painkiller medications should be used as recommended by the doctor.
  • Testicular support should be provided by wearing tight underwear.
  • Heavy exercise and physical activity should be avoided in the first days.
  • Follow-up visits recommended by the doctor should not be neglected.

Possible Risks

As with any surgical procedure, there may be some risks in Micro TESE surgery.
These include mild bleeding, swelling, pain, and temporary testosterone decrease.
When performed by an experienced surgeon, the risk of complications is quite low.

Use of Sperm After Micro TESE

The sperm obtained after Micro TESE can be used in IVF / ICSI methods.
If sperm of suitable quality and number are found, they are stored by freezing (cryopreservation) to be used in future treatments.
